Registration Requirements:
1. An appointment needs to be made with the school office for registration. We will be glad to give you a tour of the school at that time.
2. Letter of Recommendation is required for students transferring in from another school.
3. Illinois State Law requires an official record of a physical examination of all students
entering school for the first time, students entering 5th grade and students transferring
in from another school or from out of state. The record must include evidence of
immunity to those diseases specified in the recommendations of the Illinois
Department of Health along with correct dates and a physician’s signature. Health
forms must be completed and returned to the office by the first day of school. Proof
of inoculation against mumps and hepatitis must also be included. Students who do
not have this information at the office by October 15th will be suspended until the
documentation is presented.
4. New students will be admitted at the discretion of the principal and pastor after examination of the student’s records in previous schools and on the favorable recommendation of the most recent school. The record of parent involvement and cooperation will also be taken into consideration.
5. Both returning families and new ones must register each spring for the new school year. Due to mobility of families and their changing circumstances, we cannot presume or guess that a family will be able to return to school for the coming year. It is also important that we update our records and emergency information. We also need to be able to inform families of new activities and procedures.
6. Early Registration for families already enrolled in the school occur each Spring to
assure a place for children for the next school year. At that time an evaluation is
made of the present status of financial payment responsibility, and the student’s
academic, religious and social adjustment. If at that time the school discerns that it
has exhausted all available means of effectively serving the child and/or family, the
school may recommend that the parents or guardians seek another educational
environment. In the case of parishioners, such a recommendation will be made only
after consultation between the principal and the pastor of the parish.
